Scholarship Application - Autism Spectrum Disorder Foundation

We were able to send many autistic children to camp last year through our Social Skills Camp Scholarship Program which includes Aquatic Therapy and Equine Therapy.

Below is our scholarship application. Please note the following before applying:

Filling out this application does not guarantee you will receive financial assistance.
Applications are looked at in the order they are received and assistance is given based on the amount of funds available.
Any application via email, mail or phone call will not be considered. If we have questions or need additional information, we will reach out via email.
Once you have applied, please do not re-apply or email/call to check on the status. We will email you and let you know if you are receiving financial assistance.
ASDF relies on donations from the public in order to provide its valuable programs.
Payments are made by check and will be sent directly to the camp/program.
